Abirateron en prednison toegevoegd aan androgeendeprivatietherapie met of zonder docetaxel bij hormoongevoelig gemetastaseerd prostaatcarcinoom

Conclusie

In de hier besproken PEACE-1-studie wordt bij patiënten met een primair synchroon gemetastaseerd hormoongevoelig prostaatcarcinoom na een follow-up van mediaan 4,4 jaar een statistisch significant betere OS én rPFS gezien na upfront behandeling met abirateron en prednison in combinatie met standaardbehandeling bestaande uit ADT met of zonder docetaxel. De coprimaire eindpunt OS én rPFS in de populatie behandeld met docetaxel waren ook statistisch significant beter in de abirateron en prednisongroep dan in de controlegroep. Deze resultaten voldoen aan de criteria voor een positief advies volgens de PASKWIL-criteria voor palliatieve behandeling.

In de vooraf gedefinieerde subgroep patiënten met hoogvolumeziekte was de behandeling met abirateron en prednison ook significant beter voor beide eindpunten. De behandeling werd in het algemeen goed verdragen, maar ruim 10 procent van de patiënten staakte de abirateron vanwege bijwerkingen. De kwaliteit-van-levenanalyse moet nog volgen.
