Nanoliposomaal irinotecan gecombineerd met 5-fluorouracil en leucovorine als tweedelijns behandeling bij gemetastaseerd pancreascarcinoom
De NAPOLI-I-studie toont dat bij patiënten met gemetastaseerd pancreascarcinoom, na eerdere behandeling met gemcitabine voor lokaal uitgebreide of gemetastaseerde ziekte dan wel metastasering binnen 6 maanden na afronden van neoadjuvante/adjuvante behandeling met gemcitabine, de combinatie van nanoliposomaal irinotecan met 5-fluorouracil en leucovorine een 1,9 maanden langere OS (6,1 versus 4,2 maanden; HR: 0,67; 95%-BI: 0,49-0,92; P = 0,012) geeft dan behandeling met 5-fluorouracil en leucovorine alleen. Er is ook een significant effect van 1,6 maanden (3,1 versus 1,5 maanden; HR: 0,56; 95%-BI: 0,410,75; P = 0,0001) op het secundaire eindpunt PFS. Ondanks deze zeer beperkte verlenging van OS en PFS voldoen beide eindpunten aan de PASKWIL-criteria voor een positief advies. Behandeling met de combinatietherapie gaat ten koste van meer toxiciteit.
