Olaparib toegevoegd aan abirateron en prednison bij gemetastaseerd castratieresistent prostaatcarcinoom
In de hier besproken PROpel-studie wordt bij patiënten met mCRPC een statistisch significant langere rPFS gezien na eerstelijns palliatieve behandeling met olaparib gecombineerd met abirateron en prednison dan na behandeling met alleen abirateron en prednison. (HR: 0,66 [95%-BI: 0,54-0,81]; p < 0,001). De mediane rPFS was 24,8 maanden in de olaparib groep en 16,6 maanden in de controlegroep (Δ 8,2 maanden). Dit resultaat voldoet aan de criteria voor een positief advies volgens de PASKWIL2023-criteria voor palliatieve behandeling bij een mediane overleving in de controlegroep langer dan 12 maanden. Het rPFS-voordeel van olaparib is waarschijnlijk het grootst bij patiënten met een HRR-gemuteerde tumor en met name met een BRCA1/2-mutatie.
