Nivolumab en chemotherapie als eerstelijnsbehandeling van het gemetastaseerd HER2-negatief adenocarcinoom van de maag, gastro-oesofageale overgang of oesofagus
In de hier besproken CheckMate 649-studie wordt bij patiënten met HER2negatief adenocarcinoom van maag, gastro-oesofageale overgang of oesofagus met een CPS van 5 of hoger, na een follow-up van mediaan 13,1 maanden een 3,3 maanden betere OS en een 1,1 maanden betere PFS gezien na palliatieve behandeling met chemotherapie (CAPOX of FOLFOX) met nivolumab dan na chemotherapie alleen (OS: 14,4 versus 11,1 maanden; HR voor OS: 0,71[98,4%-BI: 0,59-0,86]; P < 0,0001) (PFS: 7,7 versus 6,0 maanden; HR voor PFS: 0,68 [98,4%-BI: 0,56-0,81]; P < 0,0001). Dit resultaat voldoet aan de criteria voor een positief advies volgens de PASKWIL-criteria voor palliatieve behandeling.
